Japanese encephalitis is rare in travellers but severe when it happens: around a third of people who develop the illness die, and a further third are left with lasting brain damage. Where and when
The virus is spread by mosquitoes that breed in rice paddies and pig farms, so the risk is rural rather than urban. It is found across South and Southeast Asia and the western Pacific: India, Nepal, Bangladesh, Sri Lanka, Thailand, Vietnam, Cambodia, Laos, Myanmar, the Philippines, Indonesia, Malaysia, China, Korea and Japan itself. Transmission peaks in and just after the monsoon, which in most of the region means roughly May to October.
Who needs it
The vaccine is recommended if you are:
- Staying a month or more in a risk country, particularly outside cities
- Spending significant time in rural areas during the transmission season, even on a shorter trip
- Trekking, cycling, camping, farming, volunteering or doing fieldwork in the countryside
- Visiting an area with a current outbreak
A week in Bangkok, Singapore or a beach resort does not usually justify it. A month cycling through Vietnam does.
The course
Two injections. The standard schedule is day 0 and day 28. For adults aged 18 to 64 there is a rapid schedule of day 0 and day 7. Either way, complete the course at least a week before you could be exposed.
Protection from the two doses lasts one to two years. A single booster 12 to 24 months after the primary course extends it for around ten years. The vaccine is licensed from two months of age, with children under three receiving a half dose.
Mosquito precautions still matter
The same mosquitoes carry dengue and other infections that have no vaccine. Repellent with 50% DEET, long sleeves at dusk, and a treated bed net if you are sleeping without screens or air conditioning.
Side effects
Sore arm, headache and muscle aches in a minority of people. The vaccine is inactivated.

Do I need the Japanese encephalitis vaccine for Thailand?
Not for a typical beach or city holiday. It is recommended for longer stays, rural travel, and trips in the rainy season to the north and northeast.
Can I have both doses in a week?
Adults aged 18 to 64 can, on the day 0 and day 7 schedule. Under 18s and over 65s need the 28-day schedule.
Is it a live vaccine?
No. It is inactivated, so it can be given alongside other travel vaccines and is safe for people with weakened immunity.
How long does protection last?
One to two years after the initial course, and around ten years after a booster.
